A dark-themed 3-panel simulator where 5 high-stakes tasks—from 847-recipient email blasts to $47K wire transfers—run through approval gates that fail in 4 distinct ways: HTTP 202 confusion, timeout default-allow, swallowed exceptions, and cached approval chains. Each mode shows real-time damage logging and reveals defensive Python fixes after the run. Agent Failure Series #17.
